Living Will

Create a Tennessee Living Will (Advance Directive) stating your wishes for end-of-life medical care.

A Living Will (also called an Advance Directive) lets you record your wishes about life-sustaining treatment if you become terminally ill or permanently unconscious and cannot speak for yourself.

Tennessee law (§ 32-11-105) provides the statutory form below. Two adult witnesses are required; neither may be a healthcare provider or beneficiary.
