Authorize someone to make medical decisions for you if you cannot speak for yourself.

A Healthcare Power of Attorney lets you name a trusted person (your healthcare agent) to make medical decisions if you become unable to do so.

This is different from a Living Will — your agent can make real-time decisions as situations arise, not just end-of-life decisions.

Two witnesses required. Must be notarized.
